In practice
A useful trail records the actor, the action, the target, the time, and enough of the before-and-after state to explain the change. For AI-assisted steps it should also record what the model was given and what it returned, because "why did it classify this as urgent?" is unanswerable otherwise. Entries are never edited or deleted.
Where it fits in a build
Essential in anything touching money, legal obligations, public procurement, personal data or safety. Useful everywhere else, because it is also the fastest way to debug a process complaint.
Common mistakes
- Logging the action but not the previous value, so the record shows a change happened and not what changed.
- Writing the trail to the same table as live data, where an update can quietly rewrite history.
- Recording a service account as the actor instead of the person who triggered it.
