In practice

Done well, the human step is fast: the system presents its proposal, the evidence it used and what it was unsure about, and the reviewer confirms or corrects in seconds. Done badly, it becomes a rubber stamp — a queue of approvals with no context, clicked through under time pressure, which provides the appearance of control and none of the substance.

Where it fits in a build

Put the human where the consequence is: approving an outgoing communication, confirming an extracted figure that will be submitted, signing off a classification that determines routing. Not on every step, which is how review fatigue sets in and quality collapses.

Common mistakes

• Asking for approval without showing the evidence, which makes meaningful review impossible.
• Reviewing everything, so nothing is reviewed properly.
• No record of who approved what, which leaves you unable to answer the only question that matters after a mistake.
